Rhinoplasty Course

 

Registration

Registration forms for pre-registration rates must be received by May 1.  Late registration fees are as noted on the attached form.  Registration fees include a badge, on-site program, attendance to lectures and discussions, and to exhibits.  Breakfast and lunch are included.

Cancellation Policy

Full refunds less a $75 cancellation fee will be honored before May 21, 2007.  After May 21, 2007 no refunds will be granted.  No shows are considered to be an on-site cancellation and therefore not eligible for refunds.  A cancellation requires written notification.
